We’re pleased to share our latest Annual Report, which captures the work and achievements of SWGP Federation from April 2024 to March 2025.
The report showcases the partnerships we’ve built, the services we’ve delivered, and the ways we continue to strengthen support for practices and Primary Care Networks (PCNs) across South Warwickshire.
The Annual Report provides us with an opportunity to reflect on progress, celebrate collective achievements, and share how we are responding to the evolving needs of General Practice
Highlights from this year include:
- Celebrating 10 years of South Warwickshire GP Federation
- Supported the launch, setup and delivery of five new services
- Provided support to PCNs during periods of transition, with a particular focus on workforce changes
Looking ahead, the report also sets out our priorities for the coming year. These focus on developing new services, deepening collaboration with system partners, and continuing to provide the resources and expertise that enable practices and PCNs to deliver high-quality patient care.
