Partnership working

By providing a single voice of general practice we have increased our member practices’ ability to influence the health economy and work with larger partner organisations on a more equal level.

Significant progress has been made in representing General Practice within ‘Place’ and developing a more open and transparent relationship to ensure the needs of our patients, member practices and partners are being met.

We are proud to be working on behalf of our member practices and PCNs, with our partners.

Service development progress

We are currently working with partners on the following:

Frailty service redesign

We have secured funding from South Warwickshire Foundation Trust for 8.5 (working time equivalent) Frailty Nurses. They enable care provision for patients in care homes to support the Care Home Primary Care Network Direct Enhanced Service (PCN DES).

GP in Emergency Department

We provide a GP in ED service which supports resilience in the system and helps GPs who prefer portfolio careers. With the development of the new 111 ‘talk before you walk’ campaign, this service is picking up patients that might otherwise have been referred to General Practice.
