South Warwickshire Place Partnership’s vision and priorities
South Warwickshire Place’s vision is to work together to ensure everyone is supported to live well and have safe and fully active lives. We have identified three priorities to help achieve this based on feedback and data from the local area:
- Mental health and wellbeing
Demand for mental health and wellbeing services is increasing and there is more opportunity to increase access within communities. - Cost of living
The cost-of-living pressures are an immediate risk to health and wellbeing outcomes where households do not have enough money. - Climate change
The climate is changing and having an impact on health and wellbeing and demand for services. Spikes in hot, cold and wet weather are likely to continue.
To help deliver against these priorities partners within South Warwickshire Place are working on several projects including:
- Developing a public health information sharing tool to support diabetic patients through improved education and access to a virtual Multidisciplinary Team.
- Improving respiratory health in Lillington through increasing access to health education and services and focussing on the link between housing and respiratory health. Supported by the newly developed Lillington Health Hub, a modern integrated healthcare facility and a new dedicated role.
- Increasing access to home energy and insulation grants for people with health conditions through a pilot scheme in selected town and rural areas.
- Improving mental health support for young people in Leamington and Stratford-upon-Avon through piloting dedicated roles to help the young person and their family access wider support available and to connect to local services.
South Warwickshire Place Partnership’s vision and priorities
South Warwickshire GP Federation are delivering a Child and Young Person Mental Health Care Coordinator pilot project on behalf of South Warwickshire Place. This project is taking place in Leamington North and South PCN’s, as well as Stratford PCN.
The service has been set up by health and care organisations in South Warwickshire to support children and young people while they are waiting for specialist services.
