Primary Care Networks

GP practices work together in Primary Care Networks (PCNs). These networks typically represent between 30,00-50,000 patients and are designed to help practices deliver more personalised and consistent services for their communities.

As networks, PCNs can apply for NHS funding to pay for staff in newer roles such as clinical pharmacists and social prescribers – known as ‘Additional Roles’. These roles help practices provide more personalised care to patients.

SWGP supports the seven PCNs in South Warwickshire with recruitment, contracts and training. In some cases we also provide services such as COVID vaccination clinics and evening and weekend appointments for patients.

The following practices commission some services and/or are recipients of system-wide services delivered by SWGP but are not members of the Federation.
