Primary Care Gynaecology Service

The Primary Care Gynaecology Service (PCGS) is a service we deliver in partnership with Integrated Primary Care across Coventry & Warwickshire.

The PCGS provides a single point of access for routine gynaecology conditions and treatments where these cannot be met within individual practices.

Primary Care Dermatology

Primary Care Dermatology is commissioned by Coventry & Warwickshire Integrated Care Board. The service is provided by Coventry and Warwickshire Integrated Primary Care (IPC) in partnership with Consultant Connect. South Warwickshire GP Federation provides the operational delivery on behalf of the partnership.

PCD provides quicker, more accessible specialist care for patients with dermatological conditions, at local clinics in Coventry & Warwickshire.

Dermatology

GP in ED

We provide a ‘GP in Emergency Department’ service to help improve access to urgent care and relieve pressure on A&E and GP practices.

Two GPs provide cover from 8am-12am within their own dedicated clinical area at Warwick Emergency Department. Cover is in place 365 days of the year including Christmas and other bank holidays.

This service is commissioned by South Warwickshire University NHS Foundation Trust and is a great example of different parts of the health system working together to support patients.

  • Quicker access to urgent care for patients
  • Reduced pressure on both Warwick ED and GP practices
  • Stronger relationships with acute trusts

Stop Smoking Support

We work with Integrated Primary Care (IPC) and local GP practices to provide free, tailored support that helps people in Warwickshire quit smoking for good. This service offers one-to-one support, access to stop smoking aids and resources, and specialist advice to improve your chances of successfully stopping smoking.

This service is commissioned by Warwickshire County Council.

Dermatology

Frailty nursing care

We work collaboratively with SWFT to provide a full complement of frailty nurses who receive robust training spanning primary and secondary care.

They deliver enhanced care in nursing homes, as well as supporting proactive initiatives to improve outcomes for patients and reduce demand on GP practices.

When patients under the frailty team’s care are admitted to hospital, our frailty team receives a notification so that the teams can work together swiftly to get the patient quickly and safely discharged.

Smoking Cessation Prescribing

South Warwickshire GP Federation are working with EveryoneHealth to prescribe ‘Stop Smoking Medications’ for patients registered at South Warwickshire practices.

ECO4 Project

We are working with local authorities and South Warwickshire practices to enable patients to access an energy grant via Act On Energy. We do this by confirming patient eligibility on behalf of their GP practice.

We are often called upon to support smaller-scale or ad hoc initiatives depending on system needs and PCN resources. Examples include:

Respiratory@home

Respiratory@home provides treatment and remote monitoring for patients with certain types of respiratory infections which don’t require hospital admission, including clinically vulnerable patients who are Covid positive. The service is available to patients across Coventry and Warwickshire and is provided by Integrated Primary Care. Clinically vulnerable/Covid positive patients can self-refer via https://selfreferral.happyhealthylives.uk

COVID-19 vaccine booster programme

SWGP supported Dene and Stour and Arden PCNs to deliver their spring Covid booster campaign, vaccinating eligible housebound and care home patients on behalf of both PCNs. In addition, we ran five Saturday clinics across three locations for eligible patients within Dene & Stour PCN.

We also provided HR support for Leamington PCN’s Covid programme, onboarding 27 vaccinators and admin staff on their behalf.

Acute respiratory clinics during winter 2022/23

From Jan-March 2023, SWGP quickly set up clinics for respiratory conditions at two locations in response to an urgent request from the ICB for additional capacity. This included full day clinics at weekends, improving access for patients, and reducing pressure on practices and A&E.

By responding quickly and at scale, SWGP secured additional funding to support patients within primary care.

Issuing anti-viral prescriptions for practices

Our team issued 327 anti-viral prescriptions between April 2023 and March 2024.
